If you would like to send a sample of soil, water, plant or forage to the laboratory for testing, you may download the forms to accompany your sample(s) at the following link:

You will mail the sample, form, and payment to the address on the back side of the form (where instructions for taking your sample are also located).  Your results should arrive in approximately two weeks.
